I am soon to be hosting a session with a self described "extreme pain whore". While I relish the challenge, as I have not had the pleasure of pushing a sub as far as she wants me to go, I find myself at a loss for ideas that are excrutiatingly painful but not injurious. Clamps placed in as many places as you can think to place them. Maybe see how many you can get onto each part. Maybe add some weights to these if the pain is not intense enough for her. Clover clamps are best for this - the more you pull them the more they bite.

Chemical methods - deep heat or toothpaste (toothpaste is less intense than deep heat) on the nipples and clit, fresh cut ginger root in the arse (do some research on figging). The pain starts small but builds over time and gets worse when you add water. Though some people find it less intense than others (depends on your skin sensitivity).

A combination of hot wax and ice can be exquisite. Ice can be very painful after a long exposure to it then you add the hot wax to push it to the other extreme.

One of my favorites is making little paper cuts and then pouring rubbing alcohol on them, though I don't know if that is "extreme" enough for what you are looking for (one person's extreme is another's day at the beach, if you know what I mean), though of course the level of pain relates to how deep the cut is and I don't know if that falls under "without injury" or not, though. This can also work with hot water, and it can also work with lashings left by a whip, if severe enough.

A papercut still means you get the alcohol in to the epidermis which is nice and deep, usually just deep enough to touch some nerves but not enough to bleed (which is why they hurt so bloody much) :)
